ABSTRACT

Objective To examine the effects of application of ultrasonography guided water injection for inser-tion of naso-jejunal tubes. Methods Hospitalized patients in ICU who needed naso-jejunal tubes were recruited from one tertiary hospital in Beijing from November 2016 to April 2017. Ultrasonography guided water injection was used to assist insertion of naso-jejunal tubes. Meanwhile,we conducted semi-structured interviews to learn feel-ings and suggestions from the patients. Results A total of 40 patients were included in this study,37 patients (92.5%) were successfully inserted with the tubes at the first attempt. The duration of insertion of naso-jejunal tubes was 25 (20,38.75) min. Conclusion Ultrasonography guided water injection is a simple and convenient method to guide the placement of naso-jejunal tubes for critical ill patients,which provides guarantee for early en-teral nutrition.

ABSTRACT

Objective To investigate the clinical efficacy of acupuncture at lower He-Sea point in promoting functional recovery after stomach cancer surgery.Methods Sixty patients who had undergone stomach cancer surgery were randomly allocated to treatment and control groups by using random number table method according to registration order, 30 cases each. The control group received general treatment and the treatment group, acupuncture in addition. Post-treatment gastric tube removal time, borborygmus recovery time and first fart time, the average hospitalization days, and the total cost were observed and the Quality of Life Questionnaire Core 30 (QLQ-C30) score was recorded in the two groups of patients. Post-treatment incidence of adverse reactions, peripheral white blood cell (WBC) counts, C-reactive protein (CRP) levels and plateletcrits (PCT) were compared between the two groups.Results There were statistically significant differences in post-treatment gastric tube removal time, borborygmus recovery time, first fart time and hospitalization days, and the total hospitalization expenses between the two groups of patients (P<0.01). There were also statistically significant differences in post-treatment incidences of nausea, vomiting, abdominal pain and abdominal distension between the two groups (P<0.05). Post-treatment QLQ-C30 score was (38.35±15.25) in treatment group and (76.23±10.67) in the control group; there was a statistically significant difference between the two groups (P<0.01). There were statistically significant differences in WBC and CRP levels at the end of operation and 24 hrs after treatment between the two groups (P<0.01,P<0.05). Conclusions Acupuncture can be used for early treatment after stomach cancer surgery. It can promote the recovery of gastrointestinal function and improve the overall quality of life in the patients.

ABSTRACT

Objective · To investigate the risk factors of hearing damage in child patients with severe neonatal hyperbilirubinemia (NHB) and follow up their prognosis. Methods · Clinical data of 106 newborns with severe NHB in neonatal ward of Shanghai Children's Medical Center from June 2015 to June 2016 were retrospectively analyzed. According to total serum bilirubin (TSB) level, they were divided into three groups, severe NHB group (342.0 μmol/L<TSB peak <427.5 μmol/L), very severe NHB group (TSB peak range 427.5-513.0 μmol/L), and fatal NHB group (TSB peak >513.0 μmol/L). Automatic auditory brainstem response (AABR) was used to evaluate the hearing ability of children in hospital, while those who got abnormal results would undergo diagnostic test of auditory brainstem response (ABR) when 3 months old. Auditory behavioral response of all 106 child patients at 3 and 6 months old were followed up. Results · There were totally 106 cases in three groups, among which 33 cases (33/106, 31.13%) got abnormal results at hearing screening in hospital, 22 cases (22/86, 25.58%) in severe NHB group, 9 cases (9/16, 56.25%) in very severe NHB group, and 2 cases (2/4, 50.00%) in fatal NHB group. The difference between groups was statistically significant (all P<0.05). All 33 patients not passing AABR in hospital came to undergo diagnostic test of ABR through regular education by a specially assigned person when 3 months old. Two children were diagnosed mild hearing damage. One of them was considered being caused by tympanitis. All children had good situation of auditory behavioral response at 3 and 6 months old in follow-up. Conclusion · Severe NHB is one of the high risk factors of hearing damage in neonates. The morbidity of hearing damage was higher with the increase of TSB level. The hearing damage caused by severe NHB might be reversible. It also suggested that the follow-up plan should be improved. The hearing damage caused by severe NHB might have no obvious effect on children in daily life, but long term follow-up of these children is still needed.
